The Meadows provides skilled nursing care in a warm, inviting atmosphere. Our on-site, 5-star rated Skilled Nursing and Rehabilitation Center is staffed with professionals ready to assist residents in the transition from hospital to home. We offer an array of rehabilitation services, short-term nursing care for individuals recovering from illness or injury, and long-term care for individuals needing extended nursing care.
Our community is Medicare certified and our professional staff is caring, compassionate, and available 24/7. Our goal is to help each person return to their highest level of function in a comfortable and well appointed environment.
YOUR PERSONAL CARE PLAN
Generally, individuals come to our Skilled Nursing and Rehabilitation Center directly after a hospital stay. Our licensed nursing staff and team of therapists work with each resident to develop a Personal Care Plan tailored to their individual healthcare needs, and to assist them in achieving their highest level of function and best quality of life.
This Personal Care Plan is developed utilizing a team approach, which includes the family, physician, nurses, therapists, registered dietitian, pharmacist, recreation specialist, social services, and other healthcare experts.
CORE SERVICES
Physical Therapy — Our physical therapists work with residents to improve muscular strength, balance, flexibility, and functional mobility.
Occupational Therapy — This type of therapy is designed to improve residents’ ability to perform the activities of daily living, including dressing, bathing, grooming, eating, and transferring in and out of bed.
Speech Therapy — Residents who have problems with speaking, hearing, comprehension, expression, or swallowing benefit from this type of therapy.
Restorative Therapy — Restorative therapy and services are designed to maintain function, independence, and improve self-esteem.

ACCELERATED CARE PLUS
At The Meadows, our rehabilitation therapy team is certified in an advanced therapy program called Accelerated Care Plus©. Accelerated Care Plus (ACP) is the leading provider of integrated clinical programs, therapeutic modalities, and products and services for rehabilitation.
Through the use of ACP techniques, our rehabilitation staff employs the tools and skill sets that treat a diverse range of ailments and conditions. Our therapists use a broad range of specialized techniques to extend functional gains.
ACP accelerates the introduction of traditional therapies, thus enabling more intensive therapy earlier in the stay. These techniques lead to treating pain (not just managing it), muscle re-education, wound healing acceleration, and reducing inflammation and swelling. ACP can also address the risk of future falls.
